The Zzz Hive Coaching Agreement
Coaching Agreement:
This agreement, between The Zzz Hive and/or Kristin Alexander, Rocio Zunini, Quanisha McGruder (herein named as SPECIALIST/COACH) and paid client (herein named CLIENT), for the one-on-one or sleep strategy coaching package with a fee of current price listed for 60 minutes one-on-one or current price listed for strategy package. The fee will be non-refundable after the intake form is filled out.
The service provided to the CLIENT by the SPECIALIST is infant and family sleep COACHING, as designed jointly with the CLIENT. COACHING, which is not directive advice, counseling, or therapy, may address overall goals, or general conditions in the CLIENT’s life. COACHING services may include setting expectations, education on biologically normal infant sleep, establishing goals, identifying resources, brainstorming, creating action plans (for strategy package), asking clarifying questions, and providing models, examples, and in-the-moment skills training. The SPECIALIST promises that all information provided by the CLIENT will be kept strictly confidential, as permissible by law.
Throughout the relationship, the SPECIALIST will engage in direct and personal conversations with the CLIENT, which will include asking explicit questions and making requests. The purpose of these interactions is to remind the CLIENT of his/her own intentions, and SPECIALIST him/her to realize them. In order for our relationship to achieve the maximum result, the SPECIALIST asks that the CLIENT agree to the following:
1. Please be on time for all appointments. If you will be late, notify the SPECIALIST in advance. If you will miss an appointment, notify the COACH at least 24 hours in advance. Appointments missed without 12 hours’ notice will only be rescheduled at the SPECIALIST’s discretion.
2. Be honest and participate fully and come prepared for sessions.
3. Make a commitment to the action plans you create, and do what you have agreed to do. Or solutions provided
4. ONLY FOR STRATEGY SESSION. If you see that the strategy is not working as you desire, communicate feedback to COACH to better address desired goals.
Except as expressly provided in this agreement, the SPECIALIST makes no guarantees or warranties, express or implied. In no event will the COACH be liable to the Client or Client’s family for consequential or special damages. Notwithstanding any damages that the Client and Client family may incur, the COACH’s entire liability under this agreement, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the SPECIALIST under this agreement for all services rendered up until the termination date.
This is the entire agreement of the parties and reflects a complete understanding of the parties with respect to the subject matter. This agreement supersedes all prior written and oral representations.
If a dispute arises out of this agreement that cannot be resolved by mutual consent, the Client and COACH agree to attempt to mediate in good faith for up to 30 days after notice is given.
The CLIENT’s booking and payment for this service indicates compliance with the above requests, and understanding of the services to be provided.
